Description:
Haven Health, the leading Home Health, Palliative, and Hospice Company in Southern California, is currently hiring a LVN Scheduler within our Hospice division. Responsibilities:
Coordinate and maintain daily patient care schedules to ensure timely and appropriate service delivery. Manage clinician schedules and assignments based on patient needs, orders, and availability. Coordinate staffing adjustments and promptly restaff patients in the event of clinician call-outs or schedule changes. Communicate with clinicians and internal care team members to address scheduling needs, changes, and coverage. Perform general clerical and administrative duties to support efficient hospice operations.
Requirements:
Qualifications & Requirements Fluent in Spanish and English (required).
Current California Licensed Vocational Nurse Typing:
Fast, highly accurate typist (60+ WPM). Exceptional attention to detail and data accuracy. Professional communication and customer service skills. Strong phone etiquette and ability to handle sensitive situations with empathy. Proficiency with Microsoft 365 (Outlook, Word, Excel), efax systems, and multiline phones. Experience with EMR Consolo preferred (or willingness to learn quickly). Prior hospice, home health, or healthcare administrative experience preferred. Familiarity with HIPAA and hospice documentation processes.
