: Responsible for coordinating monthly visits with provider and residential care facilities.
ACCOUNTABLE TO
: Residential Care Scheduling Lead
JOB DUTIES
Responsible for coordinating monthly visits with provider and residential care facilities in a caring and professional manner: 1. Responsible for scheduling monthly visits for patients at the residential care homes for various services such as but not limited to medical, psychiatry, podiatry, Optometry, & dental services. 2. Ensure timely delivery of daily reminders for all visits scheduled for the following day. 3. Responsible for scheduling medical necessity visits including hospital follow up, quality measures, annual physical, & vaccine visits. 4. Schedule and coordinate mobile unit visits at residential care facilities. 5. Responsible for updating master schedule and master facility list with daily updates. 6. Resolve issues involving CCCHC that arise in their assigned residential care facilities. 7. Develop strong relationships with providers via telephone and establish strong relationships with team members. 8. Provide an effective communication link between medical staff, behavioral health staff, and residential care facilities. 9. Provide customer service to residential care facilities and assigned providers. 10. Attend all required meetings and trainings. 11. Consistently demonstrate and uphold CCCHC's principle of providing quality health and human services to the medically underserved and low-income populations in a culturally sensitive manner. 12. All other duties as assigned.

Ability to exhibit good interpersonal skills. Professional appearance and demeanor. Ability to convey a positive and professional image to the public. Demonstrated proficiency in various PC applications, including E-mail, Microsoft Excel, and Word, Internet and networking devices. Demonstrated ability to learn EHR and follow instructions in that system precisely. Required to know, follow and enforce safe work practices, and be aware of company policies and procedures related to job safety, including safety rules and regulations. Education and Experience Requirements High School Diploma or GED required. Bilingual in English/Spanish required. (Other languages as necessary) Working Conditions Normal office environment with: Occasional walking, reaching with hands and arms, stooping and kneeling. Ability to handle multiple tasks while also being tactful, diplomatic, and empathetic. Ability to organize and schedule work effectively. Ability to communicate effectively and tactfully with customers, managers and other levels of personnel, in person and on the telephone. Ability to pay close attention to detail. Ability to maintain composure under conditions of urgency and/or time constraint, and meet deadlines. Ability to handle and maintain patient and company information confidentiality. Pay:
$22.00 - $24.14 per hour Expected hours: 40.0 per week
