Position Summary We are seeking a highly organized, dependable, and detail-oriented Home Health Scheduler / Intake Coordinator to join our team in Palm Coast, FL . This is an on-site position responsible for coordinating new patient referrals, verifying insurance eligibility and benefits, scheduling home health services, and communicating effectively with patients, caregivers, physicians, referral sources, and clinical staff. The ideal candidate has experience in home health, healthcare scheduling, intake, insurance verification coordination and understands the importance of timely communication and accurate documentation. Key Responsibilities Receive and process new home health referrals and intake requests. Review referral information for completeness and accuracy. Verify patient insurance eligibility and benefits. Coordinate start-of-care dates with clinical staff and referral sources. Schedule and coordinate nursing, PT, OT, ST and other home health services. Communicate with patients and families regarding visit schedules and service availability. Coordinate with physicians' offices to obtain required orders, signatures, and documentation. Monitor authorization dates, approved visits, and service periods. Follow up on pending referrals to prevent delays in patient care. Coordinate with clinical and administrative teams regarding changes in patient schedules or service needs. Identify scheduling or authorization issues and escalate them appropriately. Maintain confidentiality and comply with HIPAA and company policies. Perform other administrative duties as assigned. Qualifications Previous experience in home health, healthcare intake, scheduling, referrals, or authorizations preferred. Knowledge of Medicare, Medicare Advantage, and commercial insurance is a plus. Experience with home health system such as WellSky/Kinnser preferred. Strong understanding of insurance eligibility and authorization processes. Excellent organizational and time-management skills. Strong attention to detail. Excellent verbal and written communication skills. Ability to multitask and prioritize in a fast-paced environment. Ability to work independently and as part of a team. Professional and compassionate communication with patients and families. Basic computer skills and ability to learn new software systems quickly.
