Job Description

At Prestige Care Center of Nebraska City, we pride ourselves on providing each resident with exactly the care he or she needs and we understand that happy staff equals happy residents and our appreciation for our team is tailor-made for each individual as well. We are currently looking for an MDS Coordinator to round out our clinical team!
Position Purpose:
Conducts and coordinates the completion of required Resident Assessment Instrument (RAI) and is responsible for the implementation and ongoing evaluation of each resident's comprehensive plan of care. Required Qualifications Must possess a current, unencumbered, active license to practice as a registered nurse in Nebraska. Must have 3 years' experience as a registered nurse, one of which includes supervisory experience. Experience with MDS completion preferred. Resident Assessment Coordinator (RAC) training must be completed within six months of hire. RAC Certification is preferred. Education, training, experience, or certification in infection control and prevention. Completed specialized training in infection prevention and control through accredited continuing education.
Major Duties and Responsibilities:
MDS:
Coordination of the facility's Resident Assessment Instrument (RAI) process in accordance with state and federal regulations. Accurate completion of all MDS assessments and any supporting assessments or clinical documentation. Implementation and ongoing evaluation of each resident's comprehensive plan of care. Scheduling and leading interdisciplinary care plan team meetings. Auditing medical records for the presence of supporting documentation for all items coded on the MDS. Provide education to department heads, physicians, and other staff as needed. Collaborating with health plans, physicians, and the interdisciplinary team to ensure insurance approvals and/or that residents meet eligibility and coverage criteria. MDS/Nursing Scheduler/IP Coordinator - Job Description Interpreting rules, regulations, and coverage guidelines and acting as a primary resource for problem-solving regarding the SNF prospective payment system and quality reporting program.
Infection Prevention:
Develops and implements an ongoing infection prevention and control program to prevent, recognize, and control the onset and spread of infections in order to provide a safe, sanitary, and comfortable environment. Establishes facility-wide systems for the prevention, identification, reporting, investigation, and control of infections and communicable diseases of residents, staff, and visitors. Develops and implements written policies and procedures in accordance with current standards of practice and recognized guidelines for infection prevention and control. Oversees the facility's antibiotic stewardship program. Oversees resident care activities that increase risk of infection (i.e., use and care of urinary catheters, wound care, incontinence care, skin care, point-of-care blood testing, and medication injections). Leads the facility's Infection Prevention and Control Committee. Develops action plans to address opportunities for improvement. Participates in the facility's QAA Committee. Performs duties as assigned. Implements an annual infection control risk assessment process and participates in the facility's annual assessment review process. Reviews and/or revises the facility's infection prevention and control program, its standards, policies, and procedures annually and as needed for changes to the facility assessment to ensure they are effective and in accordance with current standards of practice for preventing and controlling infections
Schedule:
Create and post monthly staff schedules, track and post changes, and approve trades in the DON's absence or as delegated by the DON. Post and fill open shifts as they occur. Maintain timely records of call-offs, tardiness, and early arrivals, and coordinate daily with HR to ensure attendance policies are administered promptly and fairly. Provide accurate and timely reports to the DON, Facility Administrator, and management on issues relating to staffing, vacancies, agency use, or other topics as assigned. Schedule within the budget guidelines. Approve and deny PTO requests per the PTO calendar. Monitor daily PPD. Monitor and minimize staff overtime. Notify HR of terminations or no-call/no-shows as they occur to facilitate timely replacements.
