Job Description

About us Paradigm Health is a physician-led preventive and longevity medicine practice in Franklin, TN. We pair unhurried, relationship-based primary care with an on-site physical therapy division. The role Today this job is about 70% scheduling and patient access, and about 30% billing, benefit verification, and authorizations.
That mix will shift:
as our insurance volume grows, the billing and coding side will take a larger share of the week, and the person in this seat will help decide how we run it.
You own the calendar first:
our visits are longer and more involved than a typical clinic's, and getting them built correctly is what makes the day work for patients and providers alike. The rest of your week is the money side — verifying benefits, clearing authorizations, coding visits, and following up on claims. You will work directly with our operations director, physicians, and physical therapists. Real ownership here, not queue work in a large department. Scheduling and patient access Build and protect the daily schedule across physicians, physical therapy, labs, and testing rooms, keeping providers, rooms, and equipment lined up.
Coordinate our multi-part visits:
several stations in a single day, plus a lab draw a couple of weeks ahead, all of which have to fit together. Reach out to patients who are due for their next step and get them on the books — this is proactive work, not just answering the phone. Manage cancellations, reschedules, and no-shows; fill the gaps rather than letting them sit. Set up telehealth visits and make sure patients know how to join. Send confirmations and reminders and keep the calendar honest. Be the warm, competent voice on the phone. For most patients you are the first person they talk to and the one they call back. Verification, authorizations, and patient cost Verify benefits ahead of every visit through payer portals and by phone: active coverage, deductible status, copay, coinsurance, visit limits. Initiate and track prior authorizations for imaging, physical therapy, infusions, and procedures, and stay on them until they clear. Calculate what the patient will actually owe before they arrive, explain it clearly, and collect copays, deductibles, and balances at time of service. Capture and enter insurance at intake; keep coverage current in the chart. Coding, claims, and AR Code physician and physical therapy visits (E&M levels, CPT/ICD-10, timed therapy codes and units, modifiers). Prepare, scrub, and submit professional claims; work rejections, denials, and appeals. Post remittances and patient payments; follow up on AR aging. Keep payer enrollment and provider credentialing paperwork moving. Answer patient billing questions and send statements, in plain language and with patience. What we are looking for At least 2 years in a medical office, with real scheduling experience on a complex calendar. At least 2 years of billing and coding experience. Hands-on experience with benefit verification, prior authorizations, and medical billing.
Active certification required:
CPC, CPB, COC, or equivalent. Working knowledge of CPT, ICD-10, and modifiers; outpatient rehab or PT billing is a strong plus. Fluent in payer portals (e.g. Availity, Optum) and comfortable on the phone with payers when the portal will not answer. Experience with Medicare and commercial payers for both physician and therapy services. Picks up new software quickly and is comfortable being an early, opinionated user of the tools you work in every day. Unusually organized. Strict about HIPAA and patient privacy. Warm on the phone and in person. Patients will meet you. Benefits Medical insurance — practice pays 50% of the employee premium 401(k) with employer match up to 3.5% 15 days PTO, 7 paid holidays 3 paid training days per year (use them for your CEUs)
Pay:
