Provide effective communication to patients, staff members, other health care professionals, and referral sources. Communication Skills. Respond to agency and patient needs in a professional and creative manner. Demonstrate commitment, professional growth, and competency. Promote Agency philosophy and administrative policies to ensure the quality of care. Provides appropriate notification of schedule to an employee, client, contract agency, supervisor, and others as appropriate Completes documentation (paper or automated system) Responsible for monitoring and scheduling all clinician tasks and scheduling appropriately according to frequency and duration Responds to emergency calls and arranges schedules accordingly. Assigns Agency staff to clients as directed by the licensed professional staff, care plan and service request Assures employees receive complete and accurate directions and information about client needs as directed by licensed professional staff, care plans, and service request. Assist HR in disseminating the duties, Do's and Don'ts, and other policies to homemaker as mandated. Participates in the evaluation and performance review of field staff. Participates in determining hiring needs. Contact field personnel such as nurses, physical therapists, occupational therapists, and home health aides, homemaker and schedule appropriately skilled persons to staff the case. Answer, screen, Log In to report and forward incoming phone calls. Provide effective communication to patients, staff members, other health care professionals and Veterans Affairs. Respond to agency and patient needs in a professional and creative manner. Promote Agency philosophy and administrative policies to ensure quality of care. Assisting colleagues with other tasks. Take and relay messages between patients and homemakers. Effectively document patient demographics, pertinent medical history and insurance information in the patient Electronic Medical Record (EMR). Upload documents to HSRM (as needed)
