The Home Care Scheduler is responsible for coordinating daily schedules for home care patients and Patient Care Workers (PCWs). This position ensures appropriate staffing coverage, resolves scheduling discrepancies, maintains accurate patient and employee records, and provides prompt and professional communication to patients, employees, and referral sources. The position assists with home care referrals, employee hiring and orientation, and receptionion.
JOB FUNCTIONS/RESPONSIBILITIES
Coordinates daily schedules for home care patients and Patient Care Workers (PCWs). Identifies and resolves scheduling discrepancies between patients and PCWs. Maintains accurate and up-to-date spreadsheets and records for home care patients and employees. Answers incoming telephone calls promptly, professionally, and courteously. Returns telephone calls in a timely manner and follows through to resolve scheduling concerns. Communicates schedule changes and staffing needs effectively with patients, PCWs, and other staff. Processes home care referrals and inquiry calls in a timely and professional manner. Assists with applicants and the hiring process for new staff. Assists with employee orientation as requested. Greets visitors and provides professional and courteous customer service. Assists with covering receptionist breaks as needed. Maintains confidentiality of patient and employee information in accordance with HIPAA and organizational policies. Uses electronic medical records (EMR) and other computer systems as required. Performs clerical and administrative duties related to home care operations. Participates in the on-call rotation every third weekend to address urgent scheduling and staffing needs. Performs other duties as assigned.
WORK SCHEDULE AND ON-CALL REQUIREMENTS
This is a full-time position requiring40 hours per week in the officeduring the employee's regularly scheduled work hours. The position requires participation in an on-call rotation. On-call responsibilities may require the employee to respond to telephone calls, messages, scheduling changes, staffing needs, and other urgent home care matters outside of regular office hours. The employee is expected to remain accessible and respond promptly during assigned on-call periods. Additional on-call work hours will be handled in accordance with applicable organizational policies and wage-and-hour requirements.
