$18.00 - $22.00 per hour DOE Full-Time Help Keep Our Residents Connected to the Care They Need Desert Springs Post Acute is seeking a highly organized and compassionate Transportation Coordinator to join our team. In this vital role, you will coordinate resident transportation to medical appointments, treatments, and other essential destinations while ensuring a seamless experience for residents, families, and healthcare providers. Position Summary The Transportation Coordinator is responsible for planning, scheduling, and coordinating transportation services for residents while maintaining effective communication with healthcare providers, transportation vendors, residents, families, and facility staff. This role plays an important part in supporting quality resident care and ensuring appointments are managed efficiently. Responsibilities Coordinate and schedule resident transportation for medical appointments, procedures, and other approved outings Communicate appointment details with residents, families, transportation providers, and clinical staff Track and maintain transportation schedules and appointment calendars Confirm appointments and transportation arrangements to ensure timely service Monitor transportation requests and prioritize urgent needs appropriately Maintain accurate records, logs, and documentation Coordinate with clinical and interdisciplinary teams regarding resident transportation needs Assist with administrative duties and special projects as assigned Ensure compliance with facility policies and resident confidentiality requirements Qualifications High school diploma or equivalent required Previous scheduling, transportation coordination, healthcare, or administrative experience preferred Strong organizational and multitasking skills Excellent verbal and written communication abilities Proficiency with Microsoft Office and scheduling systems Ability to work independently and manage competing priorities Customer service-oriented with a professional and compassionate demeanor Experience in a skilled nursing, post-acute, or healthcare setting is a plus Why Join Desert Springs Post Acute?
