Care Manager / Care Manager Assistant Syracuse & Central New York Region Reflections Management & Care Aging Well Rochester, A Division of Reflections Management & Care Reflections Management & Care is continuing to grow throughout Central New York while expanding into the Rochester region, and we are looking for compassionate, organized, and motivated individuals to join our team in the Syracuse area. We are currently hiring for a flexible Care Manager and/or Care Manager Assistant role serving older adults and families throughout Central New York. This position is ideal for someone seeking: Flexible or very part-time hours initially Opportunity for growth into a larger caseload/full-time role over time Meaningful, relationship-based work A non-traditional healthcare role focused on advocacy, support, and problem-solving What is Care Management? Aging Life Care / Care Management helps older adults and families navigate the challenges of aging, healthcare, dementia, home care, housing transitions, hospitalizations, and long-term planning. Our team serves as advocates, coordinators, educators, and support systems for clients and families during some of the most overwhelming and emotional times in their lives.

Responsibilities May Include:

Client visits and wellness checks Attending medical appointments with clients Communicating with families and healthcare providers Coordinating home care and community resources Assisting with transitions to higher levels of care Supporting individuals living with dementia or chronic illness Documentation and individualized care planning Advocacy and crisis support

Ideal Candidates May Have Backgrounds In:

Social Work Nursing Gerontology Human Services Case Management Dementia Care Home Care or Senior Services Qualifications:

Strong communication and organizational skills Compassionate and professional demeanor Ability to work independently Reliable transportation required Experience working with older adults preferred Serving Central New York, Rochester, Finger Lakes, and surrounding regions.
