Job Description
The Case Manager is responsible for coordinating and monitoring services for individuals with intellectual and developmental disabilities (IDD). This role develops individualized plans of care, ensures services align with client needs, and serves as the primary point of contact for clients and their families. The Case Manager maintains accurate documentation, facilitates communication between service providers and community resources, participates in meetings and trainings, and ensures compliance with agency and regulatory requirements. This position also responds to client crises or emergencies, provides ongoing support through regular visits and communication, and works closely with supervisors and team members to ensure transparency and high-quality care. Clients:
Create individualized care plans, coordinate services, and be the main contact for clients and families. Provide guidance and check in regularly to make sure everyone's needs are being met.Coordinate Care:
Work with clients, families, service providers, and community resources. Keep services on track, adjust plans as needed, and make sure clients are getting what they need.Documentation & Compliance:
Keep accurate records, complete assessments and reports, and help the team stay compliant with HCS and state standards.Teamwork & Collaboration:
Work with healthcare providers and other case managers, support direct care staff, and participate in meetings and training.Crisis Support:
Respond quickly to emergencies, assess situations, and coordinate immediate action. Reach out to supervisors for after-hours issues.Communication:
Keep your team, supervisors, and leads informed so everyone is on the same page.Other Requirements:
Handle sensitive information responsibly and approach problems with a solution-focused mindset.REQUIREMENTS
Experience in case management (preferred) Experience working is individuals with intellectual and developmental disabilities (preferred) A bachelor's degree in social work, psychology, or a related field (preferred) Valid driver's license with a satisfactory driving record Successful completion of a pre-employment and annual background screening Pass pre-employment and periodic drug testing Avid Quality Care is a licensed Medicaid HCS {Home and Community-Based Services} provider that works to enhance the quality of life for mentally challenged and autistic individuals and their families by focusing on their social, emotional, physical, and intellectual needs.
