About the Center of Family Love Center of Family Love (CFL) in Okarche, OK is a faith-based, non-profit 501(c)(3) organization that provides residential and employment programs for adults with intellectual disabilities by cultivating their interests, skills and independence to live creative, productive and joyous lives. We are located 30 minutes west of north Oklahoma City.
Summary:
The Center of Family Love is currently recruiting for a Case Manager to serve as a full-time Qualified Intellectual Disabilities Professional (QIDP)
Responsibilities:
Coordinates ICF-IID resident's Individual Program Plan development and updates annually or when changes occur. Ensures completion of resident's Comprehensive Functional Assessment annually or when changes occur. Ensures completion of resident's Behavior Support Plan Annually (if necessary) or when changes occur. Prepares monthly grids for staff documentation of progress in the IPP goals and objectives. Utilizing the monthly grids, the QIDP is responsible for keeping progress notes on each resident and ensuring goals and objectives are individualized, changing when necessary. Attends monthly Human Rights Committee meetings and is familiar with the resident rights and what constitutes a restriction thereof. Attends Interdisciplinary Team Meeting and discusses the needs of ICF-IID residents on their respective case load. Consults with Administrator and DON when behaviors observed require medical intervention. Supervises all Direct Care Staff in their respective building. This includes taking call ins and scheduling of staff on each shift. Completes timely employee performance evaluation on all staff annually. Approves employee payroll. Provides coaching and training to direct care staff. This includes orientation for new hires, annual training and in-services to all staff, and disciplinary action when necessary. Ensures compliance with state regulations including requirements for temperature logs, state regulations regarding the organization and cleanliness of kitchen, pantries, supply rooms; the maintaining of dietary orders and ensuring dietary cards are correct. Completes quarterly chart audits to ensure all required documents are prepared for state survey. Ensures all residents receive recommended services including but not limited to PT, OT, ST, dental, vision, annual physicals, swallow studies, and any other need they may have. Ensures all residents have a weekly programming schedule for both in and out of building programs. Ensures residents are on time for all out of building programming including Filter Center, Art Barn, STEM, Ballet, SAIL, and any other programs added. Participates in activities such weekend cookouts, Night to Shine, Special Olympics, and Camp. Will rotate with other QIDP's with respect to weekend activities. Maintains good working relationships with family members, guardians, and friends of residents. Maintains safe and secure working environment and practices safe working habits. Other duties and responsibilities as directed.
Qualifications:
Bachelor's Degree in Education, Special Education, Psychology, Social Work or related field Minimum of one year experience working with individuals with developmental disabilities highly preferred. Strong written, verbal, and interpersonal skills. Previous experience managing staff preferred.
