This includes independent living skills, housing stabilization, household budget management, community integration, employment linkage, benefits establishment, and linkage to community providers for mental health care, substance use, and other services needed to increase housing stability. The Case Manager must create individualized goal plans and maintain accurate and current resident files. They also collaborate with community-based organizations and agencies to meet each resident's specific needs related to housing, health, mental health, and income.

Key responsibilities:

Maintain accurate records of client interactions, progress, and services provided, ensuring compliance with organizational policies. Provide immediate support during mental health crisis or housing emergencies. Act on behalf of clients to work with landlord on any housing issues, lease negotiations, etc. Connect clients with appropriate services including job coaches, mental health providers, medical providers, etc. Complete annual required trainings.

Pay Rate:

$20/hour Full-time available •Benefits are only available to full-time employees after a three-month probationary period•Medical, Dental, Vision, Life Insurance.
