Job Description

Mental Health Therapist Hiring Requirements The Therapist must have a masters degree or higher in a qualified field related to Social Work, Clinical Mental Health Counseling, School Counseling and Community Mental Health. The Therapist must have a valid driver's license with valid car insurance. The Therapist must have or be able to obtain upon hire an individual general liability insurance policy in order to practice. The Therapist must be licensed in a counseling related field such as LCSW or LPC . The Therapist must have a background free of criminal offenses in the area of insurance fraud or harm against children and consents to a background check. The Therapist must have work or internship experience related to the job duties and description below.
Duties / Responsibilities
/ Requirements Please initial by each duty / requirement Complete psychosocial assessments and treatment plans for the purpose of diagnosing mental health disorders or psychosocial factors. Provide ongoing mental health treatment to kids and families in Central OR areas assigned to or oriented on your caseload in outpatient, home or community settings. Create comprehensive treatment plans based on client's diagnosis and treatment goals. Update treatment plans at least 1x every 90 days. Utilize evidence based scoring tools to create measurably based treatment plans and goals. Complete related case notes prior to the end of the scheduled work week. Complete all billing by final work day. Accept a minimum of 1 new client per week while establishing case load or 1 new clients per month to maintain case load as assigned by Administration. Schedule own sessions on EHR software calendar. Conduct sessions in the client's home, the community or outpatient settings in order to provide continuity of care to the family system based on the family's preference. Create and maintain a crisis response plan for clients based on client need and client availability. Create safety plans with clients who demonstrate unsafe behaviors, thoughts or feelings. Keep track and upload of clinical documentation such as SUD rating scales, safety plans or other clinical records within the Theranest system. Regularly attend administrative supervision (2x per month minimum). Attend licensed provider team meeting every other week. Attend team meeting 1x per month for purposes of collaboration, supervision and support. Follows action steps created in supervision in a timely manner. Respond to the monthly team email within 3 days of its receipt with questions or concerns. Be responsive to supervisor's communication within 24 hours or next working day whichever comes first Follow the referral and insurance billing processes as outlined in the policies and procedures manual. Submit time off requests 2 full weeks prior to the first date of requested time. Work scheduled hours each week. Obtain permission to change regularly scheduled hours from Supervisor prior to changing schedule. Regularly and openly communicate with Administrator or Supervisor regarding case load, availability and challenges related to client care or work place concerns. Behave ethically, professionally and kindly with all clients, staff, community partners and supervisors at all times with no exception. Be familiar with and abide by the policies and procedures manual. Ask questions as needed and be proactive about maintaining compliance with rules & regulations. Act as a Mandated Reporter and reports suspected child abuse to the Oregon Hotline within 24 hours of learning of the suspected child abuse. Utilize Forever Family Therapy LLC documentation and forms such as assessments, CPS reporting document, safety plan, ROI's etc. as needed to support treatment of clients. Perform advocacy, environmental interventions and case management with community partners, schools or other systems the client is oriented in. Prioritize diversity, equity and inclusion work with all clients, staff and community partners. Clean up after yourself in the office settings after use. Treat all equipment (office supplies, iPads, etc.) with respect.
