Client Care Coordinator

Visiting Angels is looking for a compassionate, organized, and driven professional to join our team as an Intake Coordinator / Assistant to the Marketing Director . This role is ideal for someone who enjoys building relationships, solving problems, and making a meaningful impact in the lives of seniors and their families. You will serve as a key point of contact for clients, caregivers, and referral partners while supporting both client care coordination and community outreach efforts. ResponsibilitiesClient Care Coordination Serve as the primary point of contact for clients and their families Conduct in-home assessments and help develop personalized care plans Coordinate caregiver scheduling to meet client needs and preferences Monitor ongoing care and adjust services as needed Address client and caregiver concerns promptly and professionally Make additional home visits when necessary Maintain accurate client and caregiver documentation Assist with scheduling and coordination using AxisCare software Support compliance with company policies and procedures Follow up on leads and inquiries Discuss KPIs and performance goals with leadership weekly/monthly Encourage client and community Google reviews Marketing & Community Support Assist the Marketing Director with day-to-day activities Help coordinate and participate in community events Assist with event setup and breakdown Represent Visiting Angels at partnered community events such as Meals on Wheels and other outreach programs Additional Responsibilities Participate in on-call rotation (currently every 5th weekend and one weekday evening per week) Occasionally assist in caregiver-related duties as needed Support the mission, vision, and values of Visiting Angels Qualifications Excellent communication and interpersonal skills Strong organizational and multitasking abilities Compassionate, professional, and dependable Comfortable working independently and as part of a team Experience in home care, healthcare, scheduling, or customer service preferred Computer proficiency required; AxisCare experience is a plus Valid driver's license and reliable transportation required Benefits Meaningful work that impacts families and seniors Supportive team environment Opportunity for growth within the organization Community involvement and networking opportunities If you are passionate about helping others and want to be part of a team that truly makes a difference, we would love to hear from you.
