Key Responsibilities:

Oversee and coordinate hospice clinical operations to ensure compassionate, high-quality care. Provide leadership, mentorship, and support to clinical staff. Ensure compliance with state and federal hospice regulations . Collaborate with an interdisciplinary team to develop and implement patient-centered care plans . Participate in on-call rotation and provide weekend coverage as needed .

Qualifications:

Active RN License in the state of California ( Required ). Minimum of 5 years hospice case management experience (Required). Reliable transportation to travel between locations.
