The purpose of Co-Responder Outreach Services is to provide the Department of Social Services and Law Enforcement Agencies with access to a Co-Responder Outreach SpecialistPP, AP, QP
- who connects individuals with behavioral health needs including mental health, substance use, and intellectual developmental disabilities to available services and resources in their community.
WORK DUTIES AND TASKS
Job Duties:
Complete case management assessments to determine needs and thereafter complete a service plan. Assist individuals served with managing symptoms by identifying and addressing barriers related to appointment and medication compliance. Arrange or provide transportation to appointments if there are barriers to transportation. Assist with completing applications to services/resources if necessary. Linkage to other needed services and resources to improve areas such as behavioral health, physical health, employment, and housing. Provide education to the individual, and family as needed, about the ongoing need for outpatient services, medication management, interaction of substance abuse and medications, and overall wellness. Initiate coordination of care amongst all stakeholders, provide updates, and facilitate necessary meetings to improve the individual's care. Teach the individual how to be an active participant in services to promote holistic health care and positive outcomes by: Organizing care activities and sharing information among all the participants concerned with the individual's care. Making sure the individual's needs and preferences are known ahead of time and communicated to involved stakeholders. Educating and informing the individual about their treatment so they can be involved. Reviewing questions or ideas with the individual served that should be discussed with the provider. Teaching the individual served strategies to use as reminders (setting timer/alarm on electronic device, putting up reminder notes, etc.) for scheduled appointments/events. Perform various administrative functions related to the records and document service hours. Complete all administrative requirements in timely and efficient manner. Adhere to all clinical policies and operating procedures. Sign all treatment record notes as required and in a timely and efficient manner. Ensure compliance with uniform documentation and charting guidelines. Participate in case staffing's and other relevant meetings. Other duties as required.SCHEDULE
Fulltime:
40 hours plus per week. Fulltime Co-Responder Outreach Specialists may also be required to work On-Call hours at least one week or two weekends per month. The Co-Responder Outreach Specialist may be required to cover shifts other than their regular assigned work shift if services and extenuating circumstances deem necessary.Shifts:
1st shift 7am- 4pm 2nd shift 11am
- 8pm 3rd shift 1pm
- 10pm
QUALIFICATIONS, EDUCATION AND EXPERIENCE REQUIREMENTS A
minimum of one year's experience in providing crisis intervention services in the following settings: assertive outreach, assertive community treatment, emergency department, or other services providing 24/7 response in emergent or urgent situations AND twenty (20) hours of training in appropriate crisis intervention strategies within the first 90 days of employment. No criminal convictions of child abuse or violent crimes. Must be able to pass a criminal background, MVR and Healthcare Registry Check. Must possess a valid driver's license and basic auto insurance. Must be able to keep strict confidentiality and work with diverse populations. PP (Paraprofessional) Individual is twenty-one years of age or older and has received a high school diploma, GED, or equivalent. Upon hiring, an individualized supervision plan will be developed and supervision shall be provided by a qualified professional with the population served. AP (Associate Professional) Graduate of a college or university with a master's degree in a human service field with less than one year of full-time , post-graduate degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional with less than one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling. Supervision plan will be put in place and shall be provided by a qualified professional with the population served until the individual meets one year of experience. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually; or Graduate of a college or university with a bachelor' s degree in a human service field with less than two years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional with less than two years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling. Supervision plan will be put in place and shall be provided by a qualified professional with the population served until the individual meets two years of experience. The supervisor and the employees hall develop an individualized supervision plan upon hiring. The parties shall review the plan annually; or Graduate of a college or university with a bachelor's degree in a field other than human services with less than four years of full-time, post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional with less than four years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling. Supervision plan will be put in place and shall be provided by a qualified professional with the population served until the individual meets four years of experience. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually; or Registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing with less than four years of full-time accumulated experience in mh/dd/sa with the population served. Supervision plan will be put in place and shall be provided by a qualified professional with the population served until the individual meets four years of experience. The supervisor and the employees shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually. QP (Qualified Professional) An individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full time accumulated experience in mh/dd/sa with the population served; or A graduate of a college or university with a Master's degree in a human service field and has one year of full time, pre- or post-graduate degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has one year of full time, pre
