We are seeking a dynamic and compassionate Client Care Manager to lead our client engagement and care coordination efforts. In this vital role, you will oversee the delivery of high-quality care, coordinate services across various healthcare settings, and ensure our clients receive personalized support that promotes their well-being and independence. In this role you will be dealing with seniors, families and the heart of our company - our caregivers. Having compassion and good communication skills is key. You will spend a majority of your day interacting with our care team and families.Highly attentive and resourceful:
Able to identify needs of the client and create a care environment which allows our clients to remain in their own homes.Sharp eye for compliance:
Make sure we are following proper industry guidelines and acting in our scope of work while finding additional resources through partner resources to fulfill needs of the client. We want someone who wants to GROW with our company.What You'll Be Doing:
Answer calls, emails and messages promptly. Conduct client intakes and follow up with prospects. Schedule and perform new client assessments, including safety evaluations in their homes. Perform client reassessments on a quarterly basis or when there is a change in care. Manage client's care needs. Introduce caregivers to clients to ensure proper matching. Address client concerns and interact with family members. Interact with referral sources to ensure clients have what they need to remain safe in their homes. Represent the Assisting Hands brand in the community and foster partnerships. Assist with training caregivers in the field when necessary. Perform on-call duties as part of the office weekly rotation and act as home care aide when necessary to ensure clients receive care as promised.Knowledge, Skills, and Abilities Required:
Applicants are subject to State Licensure background checks and must have a clean driving record. Applicant must be well organized, detail-oriented, demonstrate excellent oral and written communication skills and the ability to listen effectively Maintain the desire to please clients and provide excellent customer service The position requires critical thinking skills and the ability to generate resolutions to daily challenges and opportunities Must be familiar with client care and usage of durable medical equipment that would be used in the home. Must be local to the San Francisco Bay Area area to apply Must be proficient in English both spoken and written. Reliable transportation required and willingness to travel. Availability to complete some on-call dutiesExperience:
Minimum of 1 year experience working in the home care or health care industry resulting in direct client care. Excellent communication skills and ability to engage with our caregivers and clients. Business development or sales experience preferred.Benefits:
