Client Care Manager (CCM) Be the reason someone can stay at home. This isn't just a role. It's a responsibility. You will:
Respond with empathy, patience, and genuine care Listen beyond the surface to understand what families truly need Guide them with honesty — even when that means pointing them in a different direction Because doing what's right always comes first. Create Care That Feels Personal No two clients are the same — and neither is the care we provide.
You will:
Meet families in their homes and truly get to know their story Build care plans that honor their preferences, routines, and dignity Thoughtfully match Care Pros who bring comfort, connection, and trust Be the Difference in the Details The little things matter — and you'll make sure nothing is overlooked. Conduct visits to ensure care is safe, consistent, and meaningful Address concerns before they become problems Advocate fiercely for your clients and their experience Build Something Bigger Than Yourself This role goes beyond one home, one client, one day.
You will:
Build relationships with hospitals, providers, and community partners Represent Right at Home as a trusted, compassionate resource Help expand access to care in the communities we serve Lead With Heart — Always We are a team that shows up for each other.
You'll:
Support scheduling and team needs when it counts Help welcome and guide new Care Pros Step in and provide care when needed — because leadership here means action What Makes You the Right Person for This You genuinely care about people — and it shows You communicate with empathy, clarity, and confidence You can navigate emotional situations with professionalism and heart You take ownership and follow through You're driven by purpose, not just a paycheck Qualifications High School Diploma required CNA/CMA (preferred or required based on role needs) Valid driver's license & insurance Strong communication, organization, and computer skills Basic understanding of medical terminology
