Job Description

We seek to add a compassionate and organized Home Care Client Care Manager to our growing client care team. The Client Care Manager will be responsible for ensuring the well-being and satisfaction of our clients by overseeing the delivery of exceptional home care services and training caregivers.
Starting Pay:
58,000/year
Responsibilities:
Team Leadership:
Lead and support a team of compassionate caregivers. Conduct orientation and training for caregivers before working with their clients. Provide ongoing training to caregivers on best practices and client-specific requirements.
Care Coordination:
Coordinate and schedule home care services based on client needs. Work closely with caregivers to ensure the highest standard of care is delivered.
Client Assessment:
Conduct initial assessments of clients to determine their home care needs. Collaborate with healthcare professionals to develop personalized care plans.
Client Relationship Management:
Establish and maintain positive relationships with clients and their families. Conduct regular check-ins to ensure satisfaction and address any concerns.
Quality Assurance:
Monitor and evaluate the quality of home care services. Implement improvements to enhance the overall client experience.
