Job Description

Job Overview We are looking for a compassionate and organized Case Coordinator to join our Home Care agency. As a Case Coordinator, you will play a vital role in facilitating the delivery of home care services to our clients. If you have previous experience in a similar role, possess exceptional organizational abilities, and are committed to providing excellent client care, we invite you to apply. Responsibilities Coordinate client assessments and develop personalized care plans in collaboration with healthcare professionals Schedule home visits and coordinate caregiver assignments Conduct regular follow-ups with clients and caregivers to ensure quality of care Assist in recruiting, training, and supervising caregivers Maintain accurate and up-to-date client records and documentation Coordinate with healthcare providers, pharmacies, and other service providers as needed Serve as a point of contact for clients and their families, addressing any concerns or inquiries Monitor and manage client medication schedules and refills Collaborate with insurance providers to process claims and ensure reimbursement Ensure compliance with relevant regulations and maintain confidentiality of client information Requirements Bilingual- Spanish & English Previous experience in a similar role within a home care agency preferred Proficiency in using office software and electronic health record systems Exceptional organizational and time management skills Strong interpersonal and communication skills, with a compassionate approach Knowledge of home care services, including basic medical terminology and procedures Ability to prioritize tasks and work effectively in a fast-paced environment Understanding of HIPAA regulations and commitment to maintaining client confidentiality Join our team as a Case Coordinator and play a pivotal role in enhancing the quality of life for our clients.
