Client Assessor - Home Delivered Meals Trinity Health System Steubenville , OH Clerical Full Time , M-F , Days Req # 10639 Job Summary The Client Assessor is responsible for assessing new clients and reassessing existing clients. The Client Assessor may be asked to assist in report preparation, file maintenance, and interaction with clients. The Client Assessor may be asked to provide administrative support any within the Home Delivered Meals (HDM) program. Required Education and Experience High school diploma or equivalent Valid Ohio Driver's License Associate's Degree preferred, but not required Training, Required Licensure and Certifications Some experience working in an office environment would be beneficial Thoroughness and accuracy when completing assignments Essential Key Job Responsibilities Uses an Area Agency on Aging, Region 9 (AAA9) dedicated form to assess potential clients, followed by a re-assessment annually thereafter. Notifies the HDM Coordinator of any client found to be at high risk, or found with a change in client's status based on the Client Assessment. Assists in the preparation and maintenance of HDM client files. Answers dedicated phone line to HDM department to assist clients or transfer them to the HDM Coordinator. Takes messages as appropriate and give them to HDM Coordinator. Prepares HDM reports as required. Must be efficient in the use of a computer and capable of learning new computer software.
