Job Description

Accurate Health Services is a Minnesota 245D provider delivering person-centered Home and Community-Based Services throughout the Twin Cities. We support individuals with disabilities in living safely and independently in their communities while providing responsive, culturally competent services.
Position Summary:
We are seeking an experienced 245D Program Coordinator / Designated Coordinator to join our team in a part-time hybrid role . This position combines remote administrative work with client visits, intake meetings, and occasional in-person meetings throughout the Twin Cities . The role is primarily focused on service coordination, documentation, compliance, client support, and communication with case managers and families. This is a non-direct care administrative position.
Key Responsibilities:
Coordinate and complete new client intake processes Gather and review required support plans, assessments, authorizations, and service documentation Coordinate service setup with clients, families, case managers, and staff Develop and maintain CSSP Addendums, IAPPs, risk management plans, and other required 245D documentation Track documentation deadlines, assessments, progress reviews, and service updates Monitor client outcomes and ensure support strategies are properly documented Complete client check-ins and community visits as required Communicate with case managers regarding service changes, concerns, and client needs Review documentation for accuracy and compliance Assist with incident follow-up and support plan updates Participate in internal audits and licensing preparation Collaborate with the Designated Manager on compliance and quality improvement Maintain accurate and confidential client records Assist with other program coordination and administrative responsibilities as needed
Required Qualifications:
Candidates must meet Minnesota 245D Designated Coordinator qualification requirements through one of the following pathways: Bachelor's degree in a human services field plus at least 1 year of full-time direct care experience Associate degree or diploma in a human services field plus at least 2 years of full-time direct care experience High school diploma or GED plus at least 3 years of full-time direct care experience in a human services setting In addition, candidates should have: Knowledge of Minnesota 245D services and documentation requirements Strong organization and time-management skills Strong written and verbal communication skills Comfort working independently and managing multiple client deadlines Experience using electronic documentation systems and digital forms Valid driver's license and reliable transportation Ability to successfully complete a DHS background study
Preferred Qualifications:
Previous experience as a 245D Designated Coordinator, Program Coordinator, Program Manager, Service Coordinator, or Case Manager Bachelor's degree in a human services field 2+ years of experience working with Minnesota 245D services Experience with CSSP Addendums, IAPPs, assessments, outcome tracking, and compliance documentation Experience working with culturally diverse client populations Candidates with backgrounds in 245D program coordination, case management, waiver services, service planning, residential coordination, or human services administration are strongly encouraged to apply.
Pay:
$23.00 - $25.00 per hour
