Client Care Coordinator - Home Health Right at Home - Asheville & Hendersonville, NC Position Summary The Client Care Coordinator is responsible for managing client relationships, overseeing care plans, and ensuring high‑quality service delivery. This role supports both office and field operations by coordinating caregiver schedules, conducting client assessments, and maintaining accurate documentation. You serve as the primary point of contact for clients, families, caregivers, and referral partners. Key Responsibilities Develop, update, and maintain client care plans based on health changes, family preferences, and caregiver feedback. Coordinate and adjust caregiver scheduling , matching staff qualifications and availability to client needs. Respond promptly to client and family inquiries, concerns, and service issues; escalate when necessary. Complete all required documentation, visit records, and care‑plan updates in the agency's care management system. Support intake calls and emergency service coordination. Build and maintain relationships with referral sources to support agency growth. Minimum Qualifications High school diploma or equivalent; Associate degree preferred. At least 1 year of Office experience in home health or personal care. Strong organizational, communication, and interpersonal skills. Proficiency with scheduling software and Microsoft Office/Google Suite. Valid driver's license and reliable transportation.

Managing multiple clients, caregivers, and shifting priorities.
Communication:
Serving as the central contact for clients, families, caregivers, and referral partners.
Critical Thinking:
Evaluating client needs and making scheduling or care‑plan decisions.
Documentation Accuracy:
Maintaining compliant, up‑to‑date records.
Adaptability:
Handling emergencies, last‑minute schedule changes, and evolving client conditions. This is a parttime position its hours are Monday- Friday 8a-12p. You would need to drive to our office location each day. .
Pay:
From $18.00 per hour
