Full Job Description Job Overview:
Comfort Keepers of Central Ohio is seeking a highly organized, relationship-driven professional to join our team as a Client Care Coordinator. This role plays a critical part in ensuring an exceptional experience for our clients and their families while also supporting business growth through community outreach and referral relationship development. The Client Care Coordinator guides families from their first inquiry through the start of services and maintains ongoing communication to ensure care continues to meet each client's needs. This role works closely with internal office staff, caregivers, and leadership while also representing the organization within the community to build referral partnerships and expand services. This position includes both office responsibilities and field work such as client visits, reassessments, and meetings with referral partners .Key Responsibilities:
- Manage client inquiries from initial contact through service start
- Conduct client consultations, assessments, and periodic reassessments
- Serve as the primary point of contact for clients and families regarding services and care concerns
- Maintain ongoing communication with clients to ensure satisfaction and continuity of care
- Coordinate with scheduling and office staff to ensure appropriate caregiver-client matches
- Visit client homes as needed to monitor service quality and address concerns
- Maintain accurate client records and documentation in company systems
- Resolve client concerns promptly while maintaining a high standard of service
- Develop and maintain relationships with referral sources including hospitals, senior living communities, physicians, and community organizations
- Represent the company at networking events, community meetings, and outreach activities
- Track referral sources and assist leadership in driving new client growth
- Provide feedback to leadership regarding client experience, referral trends, and growth opportunities
Requirements:
- 2+ years of experience in customer service, healthcare, admissions, sales, or a related role preferred
- Strong communication and interpersonal skills
- Excellent organizational and time-management abilities
- Ability to manage multiple priorities while maintaining attention to detail
- Comfortable working both in an office and in the field visiting clients or referral partners
- Valid driver's license and reliable transportation
- Proficiency with standard computer systems and office software
- Ability to pass a comprehensive background check and meet all screening requirements required for employment in a licensed home care agency, including a clean criminal background history consistent with state regulations.
Preferred Qualifications:
- Experience in home care, senior living, healthcare marketing, or community liaison roles
- Bachelor's degree in healthcare administration, marketing, business, or related field
