Job Summary:
Performs in-depth assessments and the formulation of individual plans of care for people with diabetes. Acts as a resource to professional staff and the community, providing outpatient diabetes education. Participates in hospital and community in-services and support groups. Investigates grant funding for projects associated with diabetes and chronic conditions associated with diabetes.
EDUCATION DESCRIPTION
Graduate from an accredited school of nursing with current license to practice in the State of Wisconsin required. Registered Dietician also acceptable. Certified Diabetic Educator (CDE) certification preferred.
EXPERIENCE DESCRIPTION
Minimum of three years of experience in field of expertise preferred.
SPECIAL SKILLS DESCRIPTION
Demonstrates understanding of teaching/learning process, nursing theory and practice. Excellent communication and demonstrated understanding of teaching/learning process, nursing theory and practice. Must be well organized, flexible, and able to work independently as well as within a team of professionals.
LICENSURE DESCRIPTION
BLS certification required.
