Job Description

Position Summary:
We are seeking an experienced and highly organized Central Admissions Director to oversee the admissions process for three Skilled Nursing Facilities, totaling more than 200 licensed beds. This leadership role is responsible for managing all incoming referrals, coordinating admissions, verifying insurance coverage, and ensuring timely communication with hospitals, referral sources, families, and facility leadership.

The ideal candidate has a strong background in skilled nursing admissions, understands Medicare, Medicaid, and commercial insurance verification, and is proficient with PointClickCare (PCC). This position requires excellent clinical review skills, strong attention to detail, and the ability to thrive in a fast-paced environment. Key Responsibilities Manage and respond promptly to all incoming referrals for three skilled nursing facilities. Review referral documentation to determine clinical appropriateness and admission eligibility. Verify insurance benefits and obtain authorizations as required. Coordinate admissions with hospital discharge planners, case managers, physicians, families, and facility teams. Maintain accurate documentation within PointClickCare (PCC) and other applicable systems. Communicate admission decisions quickly to referral sources and facility leadership. Monitor admissions pipeline and census goals while maximizing appropriate admissions. Ensure compliance with all federal, state, and company admission policies. Collaborate with facility administrators, Directors of Nursing, and clinical leadership to facilitate smooth admissions. Develop and maintain strong relationships with hospitals, physicians, and referral partners. Track referral activity and provide regular reports on admissions performance.
QualificationsRequired:
Minimum of 3 years of admissions experience in a Skilled Nursing Facility (SNF) or post-acute care setting. Experience reviewing clinical referrals and determining admission appropriateness. Strong knowledge of Medicare, Medicaid, Managed Care, and commercial insurance verification. Experience obtaining prior authorizations. Proficiency with PointClickCare (PCC). Excellent organizational, communication, and multitasking skills. Ability to prioritize multiple referrals while meeting deadlines. Strong customer service and relationship-building skills.
Preferred:
Experience in supporting admissions coordinators for multiple facilities. Knowledge of healthcare market and referral networks. Experience working with hospital case managers and discharge planners. Bachelor's degree in nursing, or a related field is preferred but
