Job Description
Job description: UMMA Senior Center is an Adult Day Health Care (ADHC) center dedicated to improving the physical, emotional, and social well-being of seniors through compassionate, person-centered care. (KOREAN
SPEAKING PREFERED
) Responsibilities Complete psychosocial assessments and participant evaluations Develop and update individualized care plans Provide counseling and emotional support to participants and families Coordinate community resources and referrals Participate in interdisciplinary team meetings Maintain accurate and timely documentation Ensure compliance withADHC/CBAS
regulations Qualifications Bachelor's or Master's degree in Social Work (MSW) Experience working with seniors or in a healthcare setting preferred Strong communication and documentation skills Schedule Part-Time or Full-Time Monday-Friday (8AM-2PM) Day shift only (No evenings or weekends) Join our caring team and help improve the lives of seniors in our community!Pay:
