QUALIFICATIONS

1. Full approval, or ability to obtain temporary approval, as a Transition Coordinator as required by the Michigan Special Education Rules (MARSE). 2. Valid State of Michigan Teacher Certificate with approval in an area of Special Education. 3. Completed a minimum of 3 years of satisfactory teaching experience in special or vocational education at the secondary level; or a minimum of 3 years of satisfactory employment providing transition related service to individuals with disabilities between the ages of 13 - 26 years. 4. Must pass criminal background check as required by School Safety Legislation.

ASSIGNED RESPONSIBILITIES SHALL INCLUDE BUT NOT BE LIMITED TO

1. Knowledge of transition foundations to help facilitate the development of educational planning, activities, and services for students, families and service providers. 2. Ability to engage in collaborative transition service delivery, and utilize interagency agreements. 3. Facilitate/teach pertinent transition practices (issues) to support special education and agency staff including: Federal/Michigan Law, Curriculum/Best Practice, Delivery of Transition Services, and Interagency Collaboration. 4. Understand outcome measurement and evaluation of transition services. 5. Work directly with teachers, MRS, counselors, Job Coach, Special Education Facilitators, students and families to assist in identifying student readiness for work-based educational experiences. 6. Collaborate with families/guardians and students to discuss and develop an appropriate Transition Plan. 7. Work with employers in developing the necessary accommodations in the work place to meet the student's individual needs. 8. Accompany students to worksites for interviews and job shadowing experiences. 9. Maintain appropriate student records, including employer training agreements and plans, performance assessments, profiles and other related documentation. 10. Provide input and implement Positive Behavior Support Plans for students, when applicable. 11. Revise tasks and provide task analysis for specific jobs/activities as it relates to student ability. 12. Participate in Individualized Education Program (IEP) meetings. 13. Provide support consistent with the student's IEP and transition plan. 14. Evaluate job sites and work-based experiences ensuring appropriateness and safety at all times. 15. Perform such other functions and duties as may be necessary and as assigned by the building principal and/or administration.
