Company Information:
Forte Residential, Inc. provides in-home support for individuals receiving services through the Medicaid Home and Community Based Waiver System (HCBS). Forte specializes in serving individuals with highly motivated family members. Our goal is to provide quality support to those we serve and their family members who devote their lives for their loved one with special needs. Forte Residential, Inc. is seeking a full-time Case Coordinator for our Columbia City, Indiana office.
Some duties include:
Training and supervising Direct Support Professionals Meeting with families of individuals with special needs Office duties Hours typically Monday through Thursday 8am-4:30pm, Friday 8am-2pm.
Some requirements include:
Some travel required, mileage reimbursed Bachelor's Degree Office skills Experience in the field (at least one year is preferred) A passion for serving individuals with special needs Some benefits include: Early Release on Fridays
