Purpose of Position:

The Service Coordinator (SC) serves as the primary point of contact for families and caregivers of children ages birth to three who are experiencing and or at risk for developmental delays in cognitive, physical, communication, social/emotional, or adaptive areas. The SC provides case management services to ensure children and their families receive the appropriate resources and services needed for optimal growth and development. This role works closely with service providers and other community partners to coordinate and implement services as outlined in the One Plan. Visits take place in the child's natural environment such as home, community-based programs, or other community settings. The Developmental Educator (DE) is responsible for developing and implementing specialized instruction for caregivers of children, ages birth to 3, who are experiencing delays in cognitive, physical, communication, social/emotional, or adaptive development. The DE models, coaches, and supports a child's caregivers to use strategies identified in the child's One Plan the help the child develop and learn. This role also involves conducting both initial and ongoing comprehensive developmental assessments to determine eligibility and monitor progress. Visits take place in the child's natural environment such as home, community-based programs, or other community settings.

Responsibilities:

• Facilitate and participate in development, review, and evaluation of the One Plan. Ensure that services are aligned with the child's developmental goals and family preferences.
• Coordinate and facilitate Initial Intake visit, Eligibility, Evaluations and Assessments, One Plan Meetings, 6-month, Annual Reviews, Transition Meetings, and additional meetings as requested.
• Work and communicate regularly with multidisciplinary teams to ensure that appropriate early intervention services are provided in a timely manner. Coordinate services across various providers to ensure the child's needs are met holistically.
• Regularly monitor the progress of children receiving services and ensure families are informed and involved in the process. Follow up to adjust plans and services as necessary.
• Build caregiver's self-advocacy skills to effectively address their basic needs, and access resources and supports to improve children's health and development
• Develop a knowledge base of community resources to share with families and providers.
• Advocate on behalf of families and children to ensure they receive necessary services. Support families in understanding their rights and available services under early intervention programs.
• Provide caregivers with accurate information about community resources. Assist in the referral process and offer guidance on how to apply for and access services/supports relevant to the child's and family's needs. Assist families to stay connected to a medical home.
• Establish and maintain relationships with local service providers, healthcare professionals, and community organizations to enhance service delivery and ensure comprehensive care.
• Receive referrals to CIS-EI when needed.
• Administer initial and ongoing all-domain developmental assessments to determine initial eligibility and continued eligibility for Early Intervention and evaluate progress.
• Provide measurable and functional outcomes and strategies which support their child's development with caregivers and team members.
• Coordinate the scheduling of evaluations, ongoing assessments, direct therapy visits, and co-visits with caregivers and team members.
• Coordinate services, develop strategies, and provide tailored, play-based interventions aimed at addressing developmental delays across multiple domains.
• Assist with sending out and reviewing parent satisfaction surveys.
• Attend 6-month annual reviews of One Plans and other team meetings, providing necessary progress reports and updated assessments. Documentation
• Maintain file on each child ensuring all documentation, releases, and consents are completed in a timely manner.
• Contributes and provides necessary documentation in the One Plan process.
• Complete Child Outcomes Summary (COS) forms
• Document all services in a timely and accurate way for billing and reporting purposes as required by Contracts and/ or PCC.
• Review and revise care plans when appropriate and necessary. Implement changes as needed.
• Assist with program evaluation, improvement plans, or other related activities as required. Professional Development
• Seek and attend professional development opportunities determined by Individual Professional Development Plan.
• Complete 10 hrs.

of professional development activities annually (CIS Institute/CIS Community of Practice Calls, On-line CIS Training Modules, etc.)

Professional Expectations:

• Maintain confidentiality and privacy while working with families. Follow mandated reporting laws as applicable
• Responsible for maintaining Vermont CIS-Early Intervention Certification
• Knowledgeable of and implement the Pyramid Model Framework.
• Maintain up to date knowledge of agency programs and services.
• Performs other duties as assigned. Qualifications Education and Experience
• Bachelor's degree in Special Education, Early Education, Early Childhood Development or Physical Therapy or Occupational Therapy, Speech and Language Therapy, Social Work, or other related human services field.
• Minimum at least 2 years of experience providing direct services with 0-3 population in Early Intervention or Early Childhood Special Education preferred.
• A minimum of two years of home visiting or related experience.
• Knowledge of healthy infant, toddler, and child development.
• Maintain Vermont CIS-EI Certification
• Knowledge of Part C regulations and VT's Special Education Rules for Part C. Skills and Knowledge
