Job Description

SUMMARY The Program Assistant works as part of an Assertive Community Team that provides care and treatment for clients that are experiencing significant mental health/psychiatric symptoms, who are demonstrating significant functional impairments, and who may have co-occurring chemical dependency. The Program Assistant is viewed as a primary agent of hope and change and is required to work in full partnership with the client in their recovery process. The Program Assistant has a two-focused level of responsibility: first, for the care, welfare, treatment, and safety of the clients as individuals and secondly, for the facilitation of a work and a treatment culture that promotes teamwork, professionalism, cultural competency, and adherence to best practice principles and interventions. Primary duties may include, assisting the ACT team with triaging scheduling needs, crisis assessment, planning and triage, coordination of care with team members and outside providers, managing the daily schedule; including the scheduling and appointment management for the psychiatric providers, assisting the team with resource acquisition and management of ongoing resource needs and providing phone support to people served. The Program Assistant plays a significant role in the daily operations of the ACT team. This person may also be assisting with making referrals, completing quality assurance activities, tracking outcomes, data entry and tracking paperwork assignments.
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below. Provides the interventions or support services as outlined by the Individual Crisis/Coaching Support Plan. Coordinates with the ACT team and other community resources. Treats clients in a respectful manner and conducts services with regard to client dignity and self-determination. Completes general office duties. Responsibility for effective intra-agency communication by responding professionally to a high frequency of calls from SpringPath clients & interfacing agencies. Adheres to Data Privacy Practices/HIPPA. Communicates, receives, and writes information clearly and accurately. Writes progress notes. Monitors and maintenance the client charts. Provides objective verbal and written critical incident analysis. Documents essential activities related to welfare and safety. Maintains service standards. Achieves client specific and program outcomes. Participates in the performance evaluation process. Increases knowledge and skills based on client and program needs. Teaches others. Seeks supervisory over-sight. Seeks professional growth. Other duties as assigned.

Education and Experience:
This position requires minimum completion of a high school diploma or GED. This position also requires the person to be 21 years of age or older.
Certificates, Licenses, and Registrations:
This position requires only that license which may be required based on education and standards determined by educational/degree guidelines. All Practitioners are supported in gaining certification as a Certified Psychiatric Rehabilitation Practitioner. The agency encourages staff to seek license at the highest standard permitted by their education and experience. Work hours are afternoon and evening. Flexibility in work hours is required. This position may require a change in hours to accommodate the programs need to have coverage (24 hours a day/7days a week). On-call rotation includes emergency response 24/7. As need dictates, it may require recall to work on short notice or to remain on duty until the need is resolved.
Other Skills and Abilities:
The Mental Health Practitioner must be able to problem-solve in a creative and independent fashion. Treatment interventions require that resolution must be sought that promotes the safety, welfare, care, and treatment of the client(s). The Practitioner must be able to intervene in a crisis in a calm, controlled, and effective manner. On-call responsibilities require assessment and problem resolution.
