SUMMARY A

MINIMUM REQUIREMENTS

Grades Kindergarten

• 3rd Arizona Early Childhood Special Education, Moderate/Severe Disabilities or Special Education ID Certificate OR Arizona Mild/Moderate Disabilities, Cross-Categorical, Special Education LD OR Special Education ED Certificate AND an Early Childhood Education endorsement OR Arizona Standard Professional Teaching Certificate AND a Full or Provisional Mild/Moderate or Moderate/Severe Disabilities PreK-12 endorsement Grades 4•5 Arizona Moderate/Severe Disabilities, Arizona Special Education ID Certificate, Arizona Mild/Moderate Disabilities, Cross-Categorical, Arizona Special Education LD or Special Education ED Certificate OR Arizona Standard Professional Teaching Certificate AND a Provisional or Full Mild/Moderate or Moderate/Severe Disabilities PreK-12 endorsement Current Arizona IVP Fingerprint Clearance Card

Proof of immunity to rubeola (measles) and rubella (German measles), or proof of MMR immunization
