Shift Differential General Description:
The Evening Critical Care Clinical Pharmacist is responsible for the implementation and provision of clinical pharmacy services to improve and enhance patient care. This position supports the inpatient units during the weekday evenings in addition to the central pharmacy operations.
Responsibilities:
Assist with order verification for inpatient hospital units. Work closely with all Pharmacy and Unit(s) personnel to ensure safe, accurate, timely, and efficient delivery of patient care Act as a liaison between the pharmacy and the medical and nursing staff for issues pertaining to clinical pharmacy services. Coordinate with decentralized clinical pharmacists to assure continuity of care Perform pharmacokinetic, antimicrobial, and anticoagulation monitoring and pharmacy consulted therapeutic management. Support the operations of the central pharmacy as needed Participate in the orientation and training of new pharmacy employees as needed. Conduct patient education for a variety of medications for patients and families (e.g. diazepam, enoxaparin, epinephrine, warfarin). Provider pharmacist oversight for medication reconciliation process and medication history technicians. Perform a variety of activities and duties as assigned to meet the routine and urgent needs of the department.
Qualifications:
Doctor of Pharmacy (PharmD) or bachelor's degree (BPharm) from an accredited college of pharmacy. Texas Pharmacist license Texas Preceptor license preferred A minimum of 1-2 years of hospital pharmacy practice experience preferred OR Completion of a PGY1 pharmacy residency program is preferred Pediatrics experience is highly preferred Compensation & Benefits For a detailed overview of our benefits, visit: Driscoll Children's Hospital Employee Benefits .
