Job Description

Job description For 30 years, School Health Clinics of Santa Clara County has offered low-cost, high-quality medical services to low-income families. Our mission is to provide high-quality, affordable primary medical care for the low-income community. Research has shown that providing low-income and minority students with healthcare and health education gives kids a chance to stay in school and perform better academically. Although we began as pediatric clinics, we now extend our services to adults. No one is ever turned away due to their inability to pay. All five clinics are located on school grounds in Santa Clara County, right in the neighborhoods where our patients live. Our staff is bilingual and multicultural so our patients can easily understand the medical advice that is given and is not intimidated because of a language barrier.
GENERAL SUMMARY
Under remote supervision is responsible for providing complete clinical care, under a physician's direction and using written protocols for qualified students who attend a designated school. Care may include both individual and/or group health teaching.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Provides comprehensive, direct medical care to children and adolescents including reproductive health using written protocols. Acts as the primary care provider for those patients who sign up for that clinic. Teaches individuals and groups about health-related topics as time permits. Performs or oversees Medical Assistants' performance of accurate CLIH-waived laboratory tests such as urine dipsticks, pregnancy, wet mounts and hemoglobin. Follows up on lab results. Dispenses pre-packaged, pre-labeled medications under protocols. Writes clear, problem-oriented medical charting on each patient seen. Serves as liaison with teachers, counselors, etc. at the school site. Provides direction to the assigned Medical Assistant. Assists in data collection for evaluation purposes on each patient seen by completing Encounter Forms. Completes necessary billing forms on each patient seen. Assures that the assigned clinic runs smoothly and in an organized manner. Oversees the day-to-day operations of the assigned clinic. Other duties as assigned. $8,000 sign on bonus available for a full time employee. $4,000 paid after 90 days and $4,000 paid after 120 days.
EXPERIENCE AND TRAINING
Certified or licensed as a Family Nurse Practitioner; and a graduate of a
NCCPA, FNP
program. B.A. or
B.S. MINIMUM KNOWLEDGE OF AND ABILITY TO
Knowledge of general and specific medical diagnoses and treatments, especially for children and adolescents; problem-oriented recording; pharmaceutical treatments; health education methods and techniques, and referral resources within the community in order to provide comprehensive treatment. Knowledge of primary, acute, and chronic healthcare management; basic laboratory work. Ability to complete written records in a timely fashion. Ability to communicate with children and adolescents; provide group and individual health teaching and counseling in order to provide high-quality physical and emotional support to patients. Ability to respond well to emergencies in a stressful environment; handle potentially volatile situations in a calm, decisive manner, and to recognize when consultation is needed
WORKING CONDITIONS
Usual clinic environment with frequent interruptions in work to answer telephones and deal with crises. May be exposed to communicable diseases.
