This energetic position offers the opportunity to work in various clinical settings, including outpatient clinics, urgent care, primary care, and specialized programs such as geriatrics, pediatrics, behavioral health, and post-acute care. As an FNP, you will leverage your clinical expertise to assess, diagnose, and manage a wide range of health conditions while fostering meaningful patient relationships. Your proactive approach and dedication will contribute to improving health outcomes and enhancing the quality of life for our patients. Responsibilities Conduct thorough physical examinations and patient assessments to determine health status and develop personalized care plans. Manage acute and chronic conditions across multiple specialties including internal medicine, pediatrics, geriatrics, and behavioral health. Administer treatments such as joint injections, IV infusions, and procedures like wart destruction, phlebotomy. Utilize electronic health record (EHR) Athena Health to document patient encounters accurately and efficiently. Perform diagnostic evaluations using laboratory testing, medical imaging, sonography, and other modalities; interpret results for informed decision-making. Provide patient education on disease prevention, medication management, nutrition, family planning, hospice & palliative care, and memory/dementia care. Collaborate with multidisciplinary teams in hospitals and outpatient clinics to ensure seamless continuity of care. Participate in clinical research initiatives and quality improvement projects to advance evidence-based practices. Assist with discharge planning, case management, utilization review, and compliance with HIPAA regulations to safeguard patient privacy. Qualifications Valid Family Nurse Practitioner license with current certification from the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC). Demonstrated experience working in diverse clinical environments such as hospital medicine, emergency medicine, outpatient clinics, nursing homes or assisted living facilities. Proficiency in EMR/EHR systems including Athenahealth (a plus); familiarity with CPT coding and ICD coding (ICD-9 & ICD-10). Strong skills in medical documentation, medication administration—including injections—and specimen collection/processing. Knowledge of physiology and anatomy principles relevant to primary care; ability to perform physical examinations and vital signs assessment accurately. Ability to work with individuals across age groups—children through seniors—and those with disabilities or complex medical needs. Excellent communication skills for patient education and interdisciplinary collaboration; commitment to maintaining HIPAA compliance and infection control standards.
