Shift Rotation:

Saturday 7:00 AM

• Monday 3:00 PM Overnight stay required

Job Overview:

We are seeking a compassionate and adaptable

CNA/QMAP

to provide care in a residential group home setting. This role combines CNA duties with medication administration responsibilities while supporting residents in a home-like environment.

Key Responsibilities:

Assist residents with activities of daily living (bathing, dressing, grooming, toileting) Administer medications as per QMAP guidelines Monitor and record vital signs and report changes Support daily routines in a residential care setting Assist residents with community activities such as appointments and errands Maintain accurate documentation and patient records Ensure resident safety, comfort, and well-being Provide individualized, patient-centered care

Additional Responsibilities:

Transport residents using a company-provided vehicle Stay overnight during assigned shifts Respond to resident needs during overnight hours Maintain flexibility in a home-based care environment

Required Qualifications:

Active CNA Certification QMAP Certification (or willingness to obtain) Strong communication and patient care skills Ability to work in a residential/group home setting Valid driving record (must pass background/DMV check)

Preferred Qualifications:

BLS/CPR Certification Compensation & Benefits:
