Monday-Friday with every-other-weekend on-call rotation Job Summary Entera Healthcare LTD is seeking an experienced, compassionate, and highly organized Hospice Director of Nursing (DON) & Compliance RN Manager to provide clinical leadership and oversee nursing operations, regulatory compliance, quality assurance, and patient care for our hospice program. This position is responsible for the overall supervision and coordination of hospice nursing services while ensuring that clinical care, documentation, staffing, and agency operations meet applicable regulatory requirements and company standards. The ideal candidate will have strong hospice nursing and leadership experience, excellent clinical judgment, knowledge of hospice regulations, and experience with HospiceMD EMR . The position is primarily based in our Rosemont, Illinois office , with field and supervisory visits required as needed. Key ResponsibilitiesDirector of Nursing & Clinical Leadership Oversee the day-to-day clinical and nursing operations of the hospice program. Provide leadership, supervision, and clinical guidance to RNs, LPNs, CNAs/HHAs, and other clinical staff. Ensure nursing services are provided according to patient needs, physician orders, Plans of Care, agency policies, and applicable requirements. Serve as a clinical resource for nursing and interdisciplinary team members. Review patient assessments, admissions, Plans of Care, physician orders, and ongoing clinical documentation. Monitor changes in patient condition and ensure appropriate clinical interventions and follow-up. Assist with complex patient cases and clinical decision-making. Oversee continuity and coordination of patient care. Coordinate clinical services with physicians, the Hospice Medical Director, pharmacies, facilities, and other healthcare providers. Participate in interdisciplinary group meetings and case conferences. Monitor admissions, recertifications, transfers, revocations, discharges, deaths, and other significant patient events. Conduct patient visits and supervisory field visits as needed. Ensure appropriate nursing coverage based on patient needs and agency operations. Provide clinical support to field staff when urgent situations arise. Illinois hospice rules address Plans of Care, hospice clinical services, nursing services, and coordination of care with the attending physician. [law.cornell.edu], [ilga.gov] Hospice Compliance & Regulatory Oversight Maintain a strong culture of clinical and regulatory compliance throughout the hospice program. Support compliance with applicable CMS, Medicare, IDPH, HIPAA, and hospice requirements . Monitor clinical practices and documentation for compliance with agency policies and regulatory standards. Assist with IDPH portal management and required regulatory submissions. Monitor regulatory changes and assist with implementing necessary operational updates. Identify potential clinical and regulatory risks. Develop and implement corrective action plans when deficiencies are identified. Monitor corrective actions to ensure successful completion. Assist with maintaining and updating clinical policies and procedures. Maintain organized compliance documentation and records. Provide compliance guidance and education to clinical staff. Illinois' hospice regulations include requirements covering administration, personnel, patient rights, clinical records, physician services, hospice program care, and quality assurance. [ilga.gov] HospiceMD EMR & Clinical Documentation Oversee clinical documentation within HospiceMD EMR . Conduct routine and focused patient chart audits. Review documentation for accuracy, completeness, consistency, and timeliness. Monitor physician orders, Plans of Care, assessments, visit documentation, and other required clinical records. Identify missing or deficient documentation and follow up with responsible clinical staff. Ensure documentation accurately reflects the patient's condition and services provided. Monitor outstanding clinical documentation. Educate clinical staff regarding proper HospiceMD EMR documentation and workflow. Maintain organized and survey-ready patient records. Work closely with clinical employees to address recurring documentation deficiencies. Quality Assurance & QAPI Participate in and oversee clinical components of the agency's Quality Assessment and Performance Improvement (QAPI) activities. Review clinical quality indicators and identify areas requiring improvement. Monitor patient care trends, complaints, incidents, hospitalizations, documentation deficiencies, and other quality indicators. Conduct internal clinical and compliance audits. Develop performance improvement initiatives. Track corrective action plans through completion. Evaluate the effectiveness of quality improvement initiatives. Report significant clinical and compliance findings to agency leadership. Promote continuous improvement, patient safety, and high-quality hospice care. Illinois hospice rules specifically include quality assurance requirements. [ilga.gov] Survey & Audit Readiness Maintain ongoing readiness for regulatory surveys, inspections, and audits. Assist with preparation for IDPH, CMS, Medicare, accreditation, and other applicable regulatory reviews . Conduct internal audits and mock survey reviews. Review required clinical and operational records prior to surveys. Assist staff in preparing for survey interviews and documentation requests. Identify potential deficiencies and implement corrective measures. Assist with the development of Plans of Correction when required. Monitor Plans of Correction and ensure appropriate follow-up. Maintain documentation supporting ongoing regulatory compliance. CMS maintains federal hospice Conditions of Participation, certification, and compliance requirements for Medicare-certified hospice providers. [cms.gov] Staff Management & Supervision Assist with recruiting, interviewing, hiring, and onboarding clinical employees. Supervise hospice nurses and other clinical staff. Provide ongoing leadership, coaching, and clinical guidance. Monitor clinical caseloads and staff workloads. Assist with clinical scheduling and staffing coverage. Conduct or participate in employee performance evaluations. Complete or coordinate clinical competency assessments. Address performance, clinical, attendance, or documentation concerns as appropriate. Implement corrective action and performance improvement measures when necessary. Monitor employee licenses, credentials, certifications, and competencies. Promote professional accountability and effective communication throughout the clinical team. Help maintain appropriate staffing to meet patient care needs. Staff Education & Training Assist with clinical orientation for new employees. Provide continuing education to nursing and clinical staff. Train staff on HospiceMD EMR documentation requirements. Educate staff regarding hospice policies and procedures. Provide education regarding HIPAA, infection prevention, patient safety, documentation, and clinical compliance. Communicate regulatory and documentation updates to clinical employees. Identify educational needs based on clinical audits and quality findings. Provide additional training and coaching when recurring deficiencies are identified. Interdisciplinary Team Responsibilities Participate in interdisciplinary group meetings and patient care conferences. Work closely with the Hospice Medical Director and attending physicians. Collaborate with nurses, aides, social workers, chaplains, bereavement staff, and other interdisciplinary team members. Communicate changes in patient condition and clinical concerns appropriately. Ensure recommendations and changes to patient care are properly coordinated and documented. Promote effective communication between office staff and field clinicians. Patient & Family Support Address clinical concerns raised by patients and families. Assist with complaint and grievance investigations when related to clinical services. Ensure appropriate follow-up on patient and family concerns. Promote respectful, compassionate, and patient-centered end-of-life care. Support the clinical team in managing complex patient and family situations. Help ensure continuity of care across home, facility, hospital, and other applicable settings. Operational & Administrative Responsibilities Work closely with the Administrator and executive leadership regarding clinical operations. Assist with clinical staffing, scheduling, and resource allocation. Monitor nursing workflow and staff productivity. Participate in management and operational meetings. Develop processes to improve clinical efficiency and quality. Support coordination between intake, admissions, nursing, compliance, and other departments. Maintain required clinical and management reports. Assist leadership with policy development and implementation. Perform additional clinical and management responsibilities as necessary to support the hospice program. On-Call Responsibilities This position is required to participate in an alternating weekend on-call rotation, every other weekend . Responsibilities during the assigned on-call weekend may include: Provide clinical guidance and support to field nurses. Respond to urgent patient care concerns and clinical escalations. Assist with arranging appropriate nursing coverage and urgent patient visits. Coordinate with physicians and the Hospice Medical Director when necessary. Communicate with patients, families, facilities, and clinical staff regarding urgent care needs. Provide clinical direction based on patient circumstances. Ensure appropriate follow-up and documentation in HospiceMD EMR . Support continuity of hospice services during weekends and after-hours periods. Conduct or assist with an urgent field visit when necessary based on patient care and staffing needs. QualificationsRequired Current and unrestricted Registered Nurse (RN) license in the State of Illinois . Minimum 2-3 years of professional nursing experience , with hospice experience strongly preferred. Clinical leadership, supervisory, management, compliance, or quality assurance experience. Strong understanding of hospice clinical documentation. Excellent clinical assessment and critical-thinking skills. Strong leadership and communication abilities. Current CPR/BLS certification . Valid driver's license and reliable transportation. Ability to conduct local field and supervisory visits. Ability to participate in the every-other-weekend on-call rotation . Preferred Bachelor of Science in Nursing (BSN) . Previous hospice leadership or management experience. Previous experience as a Hospice DON, ADON, Clinical Manager, RN Manager, Compliance Manager, or similar leadership position . Hospice compliance and regulatory experience. Experience with QAPI and quality assurance. Experience preparing for regulatory surveys and completing corrective action plans. HospiceMD EMR experience strongly preferred. Familiarity with Illinois hospice regulatory requirements. Compensation Salary:
