Bachelor's degree in Human Services, Business, or a related field, OR 3 years of experience in a related field. Minimum of 1 year of experience preferred in Incident Management, Risk Management, quantitative analysis, and reporting.Required Knowledge & Experience:
Knowledge of participant risk, including factors that place participants at risk, how risk is identified, and how risk management is applied at both the individual participant and broader population levels. Ability to apply risk management strategies to individual cases and systemically using population-based evidence. Ability to identify relationships and patterns of risk among individuals and across populations. Proficiency in case management, particularly in measures and interventions used to improve health outcomes for at-risk populations.Role Description:
Work within the Critical Incident Management Unit, Division of Participant Supports, Bureau of Coordinated and Integrated Services, Office of Long-Term Living (OLTL). Ensure quality, effective, and efficient Home and Community-Based Services (HCBS) are provided to participants through Community HealthChoices (CHC) waiver and Fee-for-Service (FFS) programs. Ensure events that place participants' health, safety, and welfare at risk are reported, investigated, and addressed according to agency policies and procedures. Ensure Critical Incident Management Unit activities align with Money Follows the Person (MFP) initiatives.Description of Duties:
Perform case reviews and record reviews to provide consultation and technical assistance to Managed Care Organizations (MCOs), Service Coordination Entities (SCEs), providers, and other agencies to assist with participant risk mitigation. Serve as a liaison between state protective service agencies, MCOs, SCEs, and internal Critical Incident Management staff to coordinate case reviews and determine whether deficiencies exist. Implement and monitor corrective actions related to identified deficiencies. Manage protective services report-of-need notifications related to OLTL program participants and providers. Coordinate information sharing with appropriate parties and provide technical assistance to other agencies. Ensure resolution of high-profile and sentinel events. Assist clinical staff and registered nurses with mortality reviews by requesting and researching case-specific information, reviewing documentation, and tracking follow-up activities. Complete trend analyses and aggregate mortality reviews by calendar year to compare findings with national health statistics and Department of Health mortality statistics. Develop and implement preventative training program proposals to address significant negative findings identified through trend analysis and mortality reviews. Work with law enforcement agencies, including the Office of the Attorney General, to explain program policies, regulations, and standards. Coordinate notifications to appropriate authorities regarding incidents of abuse and neglect of care-dependent persons. Perform data integrity activities and coordinate record correction activities for MCOs, SCEs, and providers. Participate in information system technology requirements and design sessions. Perform user acceptance testing of IT applications. Conduct data analytics by analyzing and evaluating incident report data and developing comprehensive reports. Develop, update, and refresh data reports on an ongoing basis. Utilize program applications and databases including Enterprise Incident Management (EIM), Home and Community Services Information System (HCSIS), WellSky, and Client Information System (CIS/eCIS). Utilize Excel, other statistical software, Word, and PowerPoint to support analytical, reporting, and program activities. Communicate with FFS and CHC Waiver representatives to ensure participant health and welfare. Develop and maintain tracking systems to identify patterns associated with negative participant health and welfare outcomes. Participate in workgroups, committees, and project teams related to incident management, risk management, and protective services management. Apply project management principles to assigned projects and initiatives.Essential Functions:
Maintain knowledge of medical conditions and medical terminology. Understand physical disabilities, associated limitations to Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), and relevant supports. Understand OLTL Medicaid Waivers and Act 150 programs, regulations, policies, and procedures. Demonstrate technology proficiency and the ability to work remotely using videoconference and telephonic platforms. Work effectively in a part-time teleworking environment. Work independently and demonstrate initiative. Organize and deliver presentations. Prioritize tasks effectively. Demonstrate advanced critical-thinking and problem-solving skills. Communicate effectively both orally and in writing. Understand and interpret relevant local, federal, and state laws. Possess working knowledge of, or ability to learn, Microsoft Office Suite, HCSIS, WellSky, and CIS/eCIS. Travel independently, including overnight stays, without relying on public transportation.
