• complete online and request your results to be sent electronically FBI Fingerprints (Act 114)

• includes physical (less than one year old) & tb test from less than 90 days old Act 24 (PDE Form 6004)
• Arrest/Conviction Report PA Sexual Misconduct/Abuse Disclosure Release

Proof of Pennsylvania Athletic Trainer License Proof of BOC Certification Current CPR/AED Certification Essential Duties Provide injury prevention, evaluation, treatment, rehabilitation, and emergency care services. Attend assigned practices and athletic contests. Manage concussion protocols and return-to-play procedures. Maintain medical documentation and communication with parents, coaches, physicians, and administration.
