8:00am-4:30pm. No holidays or weekend/Days/40Hours Per Week Job Summary :
Leads the NHBCS by conducting functional behavioral assessments, developing and updating Individual Behavior Plans (IBPs), delivering hands-on staff training across shifts, and coaching bedside staff to improve responses to behavioral and psychological symptoms of dementia (BPSD). Coordinates with PerformCare/CABHC, NCF leadership, family/collaterals, and prescribers to improve safety, quality of life, and measurable outcomes.Job Description:
Conduct Functional Behavioral Assessments (FBA) using direct observation, staff interviews, chart review, and collateral/family input. •Develop, implement, and maintain Individual Behavior Plans (IBPs) that are practical, unit-ready, and aligned with NCF workflows Provide on-unit coaching to CNAs/nursing staff during real care moments (bathing, meals, transitions, care refusal) Deliver role-specific training (CNAs, nurses, activities, supervisors) and "train-thetrainer" supports. Coordinate interdisciplinary case conferences with DON/Administrator, facility prescriber/medical director, and PerformCare staff Track outcomes (incident frequency/severity, ED transfers as available, staff confidence surveys) and produce monthly reports. Must be able to work flexible hours occasionally to cover high-risk time blocks and reach multiple shifts. Ability to travel locally to the NCF site(s).Education Experience:
Master of Social Work (Required) Master's degree in Social Work, Psychology or a related field. (Higher levels of training Psy.D., Ph.D will be considered but are not required.) Experience collaborating with prescribers on medication stewardship for BPSD Prior experience in SNF/NCF operations and regulatory environments.
