13326 Central Peninsula Hospital | Soldotna | AK Discharge Planning Full Time , Day , Monday - Friday 8:00am-5:oopm (exempt) This position is eligible for $25,000 relocation assistance, a Critical Position Bonus equal to $10,000 (1 year commitment) or $25,000 (2 years), and participation in the Director-level I ncentive Compensation Plan, which currently provides a maximum annual incentive opportunity of up to 10% of annual earnings.

POSITION SUMMARY

The Director of Care Coordination oversees the clinical operations of the care coordination team. Instills strong management practices to create a culture of teamwork, collaboration, and timely achievement of objectives and goals. The Director of Case Coordination develops and integrates strategies to enhancing the quality of care for patients, satisfaction, and healthcare outcomes. They not only ensure patients receive proper care, but in a cost-effective manner for the patient and their families.

REQUIREMENTS

Bachelor's Degree in Nursing or Master's Degree in Social Work 3 years of Nursing — Acute Care experience or 3 years of Social Worker experience and 1 year of experience in a progressively responsible healthcare role Alaska Licensure as a Registered Nurse or
