This position works closely with the integrated health team, medical provider, and medical assistants for care coordination. The ECD Care Coordinator will meet with community partners for follow-up updates. This position will function as "gatekeeper" for all needed referrals, both formal and informal, and for referrals that generate billable medical services, and those that do not.
Essential Functions:
Ensures all referrals are processed quickly and efficiently and according to the referral procedure guidelines. Completes Insurance verification prior to processing the referral. Provides appropriate clinical information to Specialists for referrals to be processed. Patients are notified verbally and given/sent written confirmation regarding their appointment time, location, special instructions. Utilizes and abides by proper charting and documenting procedures for all patient related services, interactions, and assignments performed. Maintains referrals tracking system and up to date referrals documentation. Follows up on prior authorizations weekly and notes made in the referral notes section statistics of all referrals processed are reported to the Operations Director/ Manager of the department for referrals not requiring prior authorization, and if patient is at a provider visit, every attempt is made to process the referral(s) while the patient is present. Maintains regular and predictable attendance. Performs other duties as required.
Position Qualifications:
Minimum Qualifications:
Bachelor's Degree in behavioral health or related field.
Preferred Qualifications:
Previous Referral experience. One (1) year of experience with behavioral health. Electronic Health Record experience.
