Overview We are seeking a dedicated and organized Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that our clients receive the highest quality of care by effectively managing their healthcare needs.

Responsibilities:

Lead and manage team of caregivers Oversee Care Plans and Care Plan Updating Scheduling Caregivers with Clients and Task Creation based on clients' needs Ensure ADPH Compliance and appropriate documentation

Experience Required:

Previous experience in senior home care required Passion and motivation to work with seniors and their families Strong organizational skills with the ability to manage multiple tasks simultaneously. Excellent interpersonal skills to foster positive relationships with clients, family members and community staff. Proficiency in office software applications to maintain records and manage scheduling efficiently.
