The Care Extender

•ECM works as a liaison for incoming Enhanced Care Management (ECM) referrals. This role engages members, screens them for ECM eligibility, coordinates referrals to Community-Based Care Management Entities (CBCMEs), and documents member dispositions in care management systems.

Responsibilities:

Manage incoming ECM referrals. Engage members and screen for ECM eligibility. Coordinate referrals with appropriate community care management entities. Document member information and outcomes accurately. Communicate with members and providers by phone and in person. Handle and de-escalate challenging calls. Work collaboratively with team members and management. Support quality initiatives and department goals.

Qualifications:

High School Diploma or GED required. Minimum 2 years of customer service experience. Teamwork experience required. Data entry experience required. Strong organizational and multitasking skills. Typing speed of 45 WPM. Proficiency with Windows applications. Excellent communication and interpersonal skills. Ability to resolve conflicts and de-escalate calls. Ability to work independently with minimal supervision. Strong patience and telephone etiquette. Ability to take initiative and complete tasks proactively.
