Help turn a plan into progress. As a Street Health ECM Care Coordinator, you'll meet people where they are - on the street, in encampments, at shelters, in clinics, and other community settings to help them navigate complex healthcare, housing, and social service systems. Working with Medi-Cal members in the field, clinics, shelters, and throughout the community, you'll coordinate services, advocate for members, and help turn individualized care plans into meaningful progress. This is a field-based role for someone who is compassionate, grounded, persistent, and comfortable working in an environment where circumstances can change quickly. The ability to meet people without judgment, build relationships over time, and stay engaged when circumstances become complicated is essential. The Role Conduct outreach and engage Medi-Cal members in street based, shelter, clinic, and other settings. Listen to members' priorities, strengths, needs, and goals and incorporate their preferences into care coordination. Facilitate referrals, appointments, transportation, and other connections to services. Communicate with healthcare providers, community agencies, housing programs, and other partners to support continuity of care. Help members understand available services, requirements, and next steps while supporting their ability to make informed decisions about their care. Track progress toward care plan goals and communicate relevant updates and concerns to the care team. Who Thrives in This Role? You may be a great fit if you: Stay calm when things are unpredictable. What You Bring 2+ years of experience in care coordination, case management, outreach, community health, or working with individuals experiencing homelessness or other vulnerable populations. Experience navigating healthcare, housing, behavioral health, and social service systems. Strong relationship-building, crisis intervention, communication, and advocacy skills. Ability to manage caseload and coordinate services in a fast-paced, field-based environment. Compensation and Benefits Pay for this position is based on experience, qualifications, and internal equity. The anticipated starting pay range is $24.00 - $27.61 per hour.
