CARE COORDINATOR
for our unit. The Care Coordinator is responsible for coordinating care for the program's patients. All positions require pre-employment drug screening and background checks.
ESSENTIAL DUTIES AND RESPONSIBILITIES
: include the following. Other duties may be assigned. Responsible for the development of care planning and patient needs assessments. Facilitates life skills groups following the CTP policies and procedures. Acts as liaison, establishing networks and collaboration with hospitals, the state, community health centers, and human service agencies about patient services and needs. Document admissions, care management needs, and discharge planning. Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake. Organize a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators. Assists treatment team with patient intake, transfer, and discharge assessments. Collaborating with the facility treatment team (medical, psychiatric, and SUD) in identifying patients' needs. From available resources, assist patients in meeting their basic needs (i.e., clothing, medical, judicial, religious, etc.).
QUALIFICATIONS
: EDUCATION and/or
EXPERIENCE
: Bachelor's degree in behavioral health and related field. The ability to obtain a Certify Addiction Technician (CAT) certification within 18 months.
OTHER :
Must have a good understanding of substance use, mental health, and treatment delivery systems. Advanced computer skills are required. CPR and First-Aid certification (initial training and renewals paid for by agency). Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary. Valid driver's license and good driving record (must provide a current unofficial DMV driving record upon hire). Able to communicate effectively in a multidimensional treatment program.
