Under indirect supervision, coordinates and provides comprehensive patient care and associated services within a defined specialty practice. Assesses patient/family needs; coordinates care and associated procedures or services for complex patients; provides education to patient/family; provides telephone advice to patient/family, based upon protocol; functions as an advocate and resource for patient/family & health care team; and facilitates communication within health care team. Mentors staff & delegates tasks to staff within the practice, collaborates with other health care providers, schools and/or community services to meet patient/family needs. Education and/or

Experience Required:

Graduate of an accredited Registered Nurse Program. BSN preferred Minimum 5 years pediatric nursing experience preferred. Experience in working in clinical or community setting with children and families preferred. License and/or

Certification Required:

Current State of Connecticut Registered Nurse licensure required and maintained thereafter Current Basic Life Support (BLS) / CPR in adult, child & infant required and maintained Specialty licensure/certification may be required, as specified by department.

Clearly documents all pertinent aspects of care, including: a plan of care; a regular summary of patient status and family adjustment; unexpected events of changes in patient condition; medications; and monitoring of vital signs, etc. Complies with CCMC policies regarding appropriate documentation. Adheres to current HIPAA Guideline to insure patient and family confidentiality is maintained. Delegates aspects of care to peers and ancillary personnel to meet needs. Serves as a resource to staff within area of expertise. Networks with healthcare team by documenting and communicating comprehensive data including complex child/family needs to provide continuity. Demonstrates knowledge of the age-related differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families. Values cultural diversity and other individual differences in the workforce.
