We are seeking a compassionate, organized, and motivated Care Coordinator to join our team for a temporary full-time assignment. The selected candidate will participate in structured training and work closely with experienced professionals to develop the skills needed to coordinate high-quality, person-centered care. Position Summary As a Care Coordinator, you will serve as an advocate and resource for residents by helping coordinate services, connecting individuals with community resources, and collaborating with treatment providers, families, and interdisciplinary teams to support recovery and long term success. This position provides valuable experience for individuals interested in behavioral health, social services, counseling, psychology, human services, or related fields. Essential Responsibilities Coordinate services and supports for assigned residents. Develop and monitor individualized goal plans. Collaborate with residents, families, healthcare providers, and community agencies. Connect residents to community resources and support services. Maintain accurate, timely, and confidential documentation. Monitor resident progress and assist with goal planning. Participate in interdisciplinary team meetings. Advocate for residents while promoting independence and recovery. Assist with crisis intervention and problem-solving as needed. Ensure compliance with agency policies and applicable regulations. Minimum Qualifications Bachelor's degree in Human Services, Psychology, Social Work, Counseling, or a related field OR Minimum of three (3) years of experience working in the mental health or behavioral health field. Valid driver's license with reliable transportation. Excellent written, verbal, and interpersonal communication skills. Ability to pass all required background screenings. Preferred Qualifications Experience in case management or care coordination. Knowledge of behavioral health systems and community resources. Experience developing individualized goal plans. Strong organizational and time management skills. Ability to prioritize multiple responsibilities. Experience working with multidisciplinary teams. Knowledge of documentation standards and confidentiality requirements. Strong critical thinking, crisis management, and problem-solving skills.
