Job Summary:
Responsible for coordinating the obstetrics department by making sure that all of the steps necessary for the process of all prenatal care and post pardon care and follow up are functional and ongoing.Primary Responsibilities and Specific Duties:
- Participates in patient care coordination, pre-visit planning, huddles, one-on-one communication, email, patient contact, and documentation of any services accessed and/or health care related concerns/needs that have arisen since last visit.
- Participate in team meetings to communicate and address patient care and operational requirements.
- OB preliminary intake
- Take patient's OB history.
- Do HIV pre-counseling.
- Develop statistics on utilization of services and numbers of high-risk cases and other relevant information.
- Track all compliant and non-compliant and high-risk OB patients. oCreate Actions for each OB patient
- Collect and enter data for the UDS prenatal report.
- Healthy start prenatal risk screening
- Ensures screenings are completed on every patient.
- Ensures information is forwarded to Orange County Healthy Start
- Other duties oOB floor nurse as required.
- Perform other duties as assigned or necessary.
