Case Management Care Coordinator | Case Management | Day | Full-Time

Job Description

Overview The Case Management Care Coordinator assists in the clerical functions of the discharge planning process; the coordination, collection and recording of Case Management information in compliance with Medicare, Medicaid, and third party payers; ensuring discharge needs are met and that the patient and their families understand the availability of resources for their medical and social support and education.
Qualifications Education:
High School Diploma or GED equivalent.
Experience Required:
Must be able to read, write, speak and understand English. - Computer operation, organizational skills, ability to communicate well and prioritize, prior health care experience
